What is RECOVIA?

RECOVIA is a unique wholistic program aimed at helping individuals improve function and quality of life. Outcomes have shown patients that do the best in program where highly functional at one point in their life but now find themselves struggling to regain that function. This could be impacted by many issues including general mental health challenges, chronic pain, medication or substance use. At Recovia the "something" does not matter as much as all the things that can be put together under one roof to create a comprehensive plan customized for each patient. Call today for an evaluation.

What is the first step in the process?

Scheduling is easy! Call and speak to one of our team members today at 480-712-4600 to learn more about our unique treatment model and if it is a fit for you. Or, fill out an online submission form and we will get back to you within 24 hours. Either way, our team is here to help.

What to expect on your first day of the program?

Your first day with us in Active Care will be mostly informational and will introduce you to the process. You will have the opportunity to meet individually with each provider, learn the layout of the facility, and get comfortable as you begin your program. Our team will guide you through the process from start to finish.

What do I wear on my first day of program?

The Recovia Program involves light physical activity. Please wear comfortable clothing (active wear) and tennis shoes.

What do I need to bring with me to the program?

A small locker will be provided to each patient while at the facility. You are welcome to bring headphones, a book, or personal items that you would like to use during your downtime.

If you are taking medication: please bring all medications with you to your first appointment.

Please bring your own snacks and a water bottle, a small refrigerator is available to keep your snacks/water cold during the day.

If I choose to Detox, what will that look like?

Each patient will individually meet with the Medical Provider to discuss his/her detox process and other substances. In general, we use MAT (Medication Assisted Treatment) to transition patient off opioids or other substances. Additional comfort medications will also be prescribed if needed. Our goal is to create a safe, smooth and comfortable detox process for every patient. All detox treatment plans are outpatient based.

How long is my program?

All programs are determined after the initial evaluation. Each evaluation includes behavioral health, physical therapy and a medical review. Your primary provider will contact you after your eval to review your team's recommendations.

What insurance plans do we take?

I do not want to commit to a full program, can I come for Medical Management/Detox?

The Recovia program follows the guidelines for MAT (Medication Assisted Treatment) which includes a combination of medication and behavioral therapies. Our integrated program is effective due to the combination of Physical Therapy, Behavioral Health Counseling and Medication Management.

Does RECOVIA Offer Payment Plans?

Seeking treatment can place a financial burden on our patients and their families, but we feel that this should not be a barrier to joining the Recovia program.

For those who are unable to pay for treatment in one payment, Recovia offers payment plans that can be split into multiple payments and are based on the financial ability of our patients and/or their families.

To establish a Recovia payment plan, patients will be asked to review and sign a payment agreement. To acquire a payment agreement or to learn more about our payment plans, please contact our Patient Relations department at 480-308-1729.

How To Refer A Patient

Applying for, or referring a patient to the Recovia Functional Flex-care Program can be done in any of the following ways:

  • Online
    Complete and submit the online application by using the program application form to the right. Our patient coordinator will review the application and contact you following submission receipt.
  • Fax
    Please complete the Recovia Referral Form and send medical records by fax to 602-428-7045. The Recovia team will review the referral information and will schedule a Multidisciplinary Evaluation.
  • Phone
    Call 480-308-1729 to speak with our Patient Coordinator.
  • Facility Tours
    Facility tours are always available on a scheduled basis. If you are interested in touring the Recovia facility, or simply learning more about the program, please contact us at 480-308-1729.
